Baranovichi city hospital health care institution was established by Decision No.41 as of 17.01.2006 “On the spin-off of Baranovichi central outpatient clinic” and was registered in the Unified Register of Legal Entities and Individual Entrepreneurs on 13.02.2006, entry No. 290453318.

All rights and obligations of Baranovichi central outpatient clinic shall be transferred to Baranovichi city hospital in accordance with the spin-off balance.

Submission and consideration of the applications of the citizens and legal entities

Baranovichi City Hospital considers the applications of the citizens, including individual entrepreneurs and legal entities, in accordance with the Constitution of the Republic of Belarus, Law “On the applications of the citizens and legal entities” as of July 18, 2011, Decree of the President of the Republic of Belarus No. 498 “On the additional procedures for the consideration of the applications of the citizens and legal entities” as of October 15, 2007, Directive of the President of the Republic of Belarus No.2 “On red tape reduction and better social support” as of December 27, 2006 and Law No. 2435-XII “On health care” as of June 18, 1993.

All written applications shall be sent to the following address: 225409, Brest Region, Baranovichi, 133 Brestskaya st., UZ Baranovichi City Hospital.

The text of the application shall be readable and legible. It shall not contain any offensive or foul words or expressions.

An applicant’s representative shall attach a document, confirming its right of representation, to a written application.

An application shall be made it the Russian language or the Belarusian language in accordance with Law “On the applications of the citizens and legal entities” as of June 18, 2011.

An application shall not be considered if:

  • it is made neither in the Russian language nor in the Belarusian language;
  • an applicant fails to state its surname, first name, patronymic, address of residence or stay;
  • an applicant fails to state a full name of a legal entity or its location address; surname, first name or patronymic of a chief or a person duly authorized to sign applications (for legal entities);
  • it is illegible;
  • it contains offensive or foul words or expressions;
  • the application is to be considered in accordance with the constitutional procedure laws, civil laws, civil procedure laws, commercial procedure laws, criminal procedure laws, administrative procedure laws, or if the application is to be submitted and considered in accordance with any other procedure stipulated by the legal acts;
  • it contains the issues which are not within the competence of Baranovichi City Hospital;
  • an applicant fails to submit a complaint on time for no good reason;
  • it is a second application and if a previous application has already been considered on the merits, provided that the second application does not contain any new important information for the consideration of such application on the merits;
  • communications with an applicant were terminated.

An applicant shall be entitled to withdraw its application before it is consideration on the merits by providing a written request hereof.

An applicant shall be entitled to appeal against the answer of the health care institution or its decision to reject an application in accordance with the procedure stipulated by Article 20 of Law On the applications of the citizens and legal entities” as of June 18, 2011.

Online applications

On applications of the citizens, including individual entrepreneurs and legal entities, shall be submitted to and considered by UZ Baranovichi City Hospital in accordance with the Law On the applications of the citizens and legal entities” as of June 18, 2011.

Please note that on applications sent to any other e-mails (for e.g. e-mails of subdivisions or employees’ e-mails) will not be considered in accordance with the Law On the applications of the citizens and legal entities” as of June 18, 2011.

An on application for the citizens shall include:

  • surname, first name, patronymic (if any) or initials;
  • address of residence or stay;
  • essence of the issue
  • citizen’s e-mail address.

An application for a legal entity shall include:

  • full name of a legal entity and location address;
  • essence of the issue;
  • surname, first name and patronymic (if any) or initials of a chief or a person duly authorized to sign applications;
  • e-mail address of a legal entity.

If an on application is submitted by an applicant’s representative, it shall attach copies of the documents confirming its authorities.

An applicant shall be entitled to withdraw its application by providing a written request or an on request (in the same way as an on application).

An applicant may attach any documents and (or) information (documents confirming the representatives’ rights, documents concerning previous applications, and other documents and (or) information required to solve the issues set forth in the applications) to the on application form (in valid formats).

An answer (notification) to an on application shall be sent to the e-mail address, specified in the on application, or to a citizen’s address of residence or stay, or to an address of location of a legal entity if

  • an applicant asks to send a written reply to its address or to sent a written reply both to its address and e-mail;
  • a sender fails to send a reply (notification) to the e-mail specified in the on application for technical reasons;

If numerous (more than ten) applications on similar issues from different applicants are submitted, replies to such applications may be posted on the official websites of government bodies or other government authorities competent to consider such issues, on the internet. In this case no replies (notifications) will be sent to applicants.

Reception Hours

The Chief Physician or the Deputy Chief Physician of Baranovichi City Hospital receives the citizens, including individual entrepreneurs, their representatives, representatives of legal entities (hereinafter referred to as reception of citizens) on personal matters in accordance with the schedule approved by the Chief Physician of Baranovichi City Hospital.

A citizen, individual entrepreneur or a legal entity may schedule an appointment by a written application, online application, schedule an appointment in-personal or by phone (8 (0163) 63-23-35). An appointment shall be made from 08.00 a.m. to 01.00 p.m. and from 0.30 p.m. to 04.30 p.m. on working days. An applicant shall describe the problem and provide any required information.

To make an appointment an applicant shall provide an identification document (passport, residence permit or refugee identity card). An applicant’s representative shall also provide a document, confirming their right of representation.

If a reception day falls on a public holiday or a day-off declared by the President of the Republic of Belarus, a reception day shall be the next working day.
